Products marked “compounded” are not FDA-approved. The FDA does not review compounded drugs for safety, effectiveness or quality. All treatments require an evaluation by a licensed provider.
Every provider below uses US-licensed clinicians and pharmacies.
Four things to understand before you start an online visit.
Generic sildenafil and tadalafil contain the same active ingredients as Viagra® and Cialis® and usually cost far less.
Sildenafil is taken before sex. Low-dose daily tadalafil is an option for some men — your provider will help decide.
Chews, mints and combination formulas are often compounded: legal when prescribed, but not FDA-approved.
ED medication is not suitable with nitrates or certain heart conditions. Answer every health question honestly.
Erectile dysfunction is a common concern that affects men of all ages. Whether it is trouble keeping an erection, lower confidence or changes linked to health and ageing, finding help that feels reliable and private can be hard. We reviewed the leading online ED treatment platforms of 2026 and built this comparison so you can explore your options with confidence.
The treatments featured here are offered through licensed providers and can include prescription medication, personalised care plans and online consultations — discreet, professional support from home.
Erectile dysfunction (ED) is difficulty getting or keeping an erection firm enough for sex. It is very common: research suggests roughly 40% of men have experienced it by 40, and close to 70% by 70.
The good news is that ED is one of the most studied conditions in men's health, and there are many effective treatments. Premature ejaculation — another common concern — is usually treated separately, with its own medications and techniques.
Nearly every man will notice ED symptoms at some point. Common signs include:
ED is not always permanent. Symptoms can come and go, or become more persistent over time. Diagnosis usually starts with a conversation with a healthcare provider; if medication does not help, a physical exam or further tests may be recommended to find an underlying cause.
ED often has more than one cause. Physical factors include narrowed arteries (linked to high blood pressure, high cholesterol, diabetes and atherosclerosis), obesity, low testosterone, smoking, heavy drinking and some surgeries or injuries. Psychological factors include stress, anxiety, depression, relationship problems and performance anxiety.
Medications can also contribute, including some antidepressants (SSRIs and SNRIs), antipsychotics, and older blood-pressure drugs such as beta-blockers and diuretics. Never stop a prescribed medicine without speaking to your doctor — there are often alternatives.
Most oral ED medications are PDE5 inhibitors. They block the PDE5 enzyme, which increases blood flow to the penis in response to sexual stimulation. They do not create desire or raise testosterone — they support the physical response needed to get and keep an erection.
Be wary of products sold as “male enhancement pills” or “the best sex pills”. The medically recognised options are prescription medications that improve blood flow. Healthy habits — good sleep, a balanced diet, exercise, managing stress and communicating with your partner — can support both desire and erectile function.
It depends on the medication. Sildenafil and vardenafil usually work in about 30–60 minutes and last around 4–6 hours. Tadalafil can start in 30 minutes to 2 hours and last up to 36 hours. Some sublingual and rapid-dissolve formats are designed to work faster. Always follow your provider's dosing instructions.
No. Premature ejaculation (PE) means climaxing sooner than you would like, while ED is trouble getting or keeping an erection. PE treatment focuses on delaying climax — behavioural techniques, prescription medication or topical numbing products — while ED medications target blood flow. Some men experience both, and a provider can build a plan that addresses each.
Prescription pills. PDE5 inhibitors such as sildenafil, tadalafil and vardenafil are FDA-approved and widely prescribed. They are not aphrodisiacs, but they help men with ED achieve firmer, longer-lasting erections.
Generic medications. Generics contain the same active ingredients as brand-name drugs and meet the same FDA standards for safety, quality and effectiveness — usually at a much lower price, sometimes just a few dollars per pill.
Over-the-counter products. There are no FDA-approved oral ED pills sold over the counter. Supplements sold in shops and online as “miracle cures” can be ineffective or even unsafe, and some have been found to contain hidden drug ingredients.
Testosterone therapy. Testosterone naturally declines with age. Diet, exercise, sleep and vitamin D can help, but testosterone replacement therapy (TRT) is only prescribed for diagnosed low testosterone and is not a standard ED treatment by itself.
Vitamins and supplements. Nutrients such as vitamin D, B vitamins and L-arginine/L-citrulline (which support nitric oxide production) are often discussed, but evidence is mixed and they are less effective than prescription medication. Too much of some supplements can be harmful or interact with medicines.
Side effects. ED medications are usually well tolerated. Common side effects include headache, flushing, indigestion, nasal congestion, back pain and mild vision changes. A provider can adjust the dose or medication if side effects bother you.
Topical gel. Eroxon is an FDA-authorised gel available without a prescription that works through a cooling and warming effect. Some men report skin irritation, and contact with a partner is possible.
Vacuum pumps. A pump draws blood into the penis and a ring at the base helps maintain the erection. Results are temporary, and the ring should be removed after about 30 minutes.
Shockwave therapy. Low-intensity shockwaves aim to stimulate new blood vessel growth. It is mainly considered for persistent ED or men who do not respond to pills, and is usually performed in a clinic.
Injections. Self-administered penile injections work quickly and reliably, and are usually reserved for men who do not respond to oral medication.
Lifestyle changes. Regular exercise, a healthy diet, less alcohol and quitting smoking can improve symptoms and overall sexual health.
For many men, ED can improve or even resolve. Research suggests around a third of men experience remission within five years, especially when the underlying cause is addressed. Lifestyle changes, therapy for performance anxiety and medication all play a role in managing or reversing the condition.
Please note: Side effects of ED medication are usually mild. Always follow a licensed healthcare provider's guidance before starting any new medication or supplement.

Erectile dysfunction is the ongoing difficulty getting or keeping an erection firm enough for sex. It is common, especially with age, and can be linked to heart health, diabetes, blood pressure, medications, stress or lifestyle. A licensed healthcare provider can help identify the cause.
Yes. In the United States, licensed telehealth providers can evaluate you through an online questionnaire or video visit and, if appropriate, issue a prescription that is filled by a licensed pharmacy.
Yes. FDA-approved ED medications such as sildenafil (the active ingredient in Viagra®) and tadalafil (the active ingredient in Cialis®) are prescription-only. Every provider listed here requires a medical review by a licensed clinician before treatment.
FDA-approved generics contain the same active ingredient, strength and dosage form as the brand-name drug and must meet the same quality standards. They usually cost much less.
Compounded products (for example some chews, sublingual tablets or combination formulas) are prepared by pharmacies for individual patients. They are not FDA-approved, which means the FDA has not reviewed them for safety, effectiveness or quality. Ask your provider about the risks and benefits.
It can be, when you use a licensed provider and pharmacy and answer the medical questionnaire honestly. ED medications are not safe for everyone — for example, people taking nitrates or with certain heart conditions. Never take ED medication without a prescription.
Common side effects include headache, flushing, nasal congestion, indigestion and back pain. Seek emergency care for an erection lasting more than 4 hours, sudden vision or hearing loss, or chest pain.
